I set my HP and MP potions to "PageUp" and "PageDown" respectively. When I logged in today, the potions were not on the assigned hotkey anymore, while the other keys remained (Insert, Home, Delete, End)
I've had a similar issue. I put my main attack skill on the Shift key, but it's always removed whenever I relog. Same goes for my potion on my Delete key, but the one on Insert stayed.
